MAT Application Checklist
Please observe the following checklist to complete your application:
- Obtain student copies of your college transcripts to assist you in completing the coursework section for the application on the 《十大菠菜靠谱老平台》 site.
- Create an account 和 sign in to 《十大菠菜靠谱老平台》.
- Review the 《十大菠菜靠谱老平台》 Instructions & 常见问题信息. Identify school(s) to which you’d like to apply.
- Using the 《十大菠菜靠谱老平台》 transcript request form, request official transcripts from each institution you have attended. 填写表格, submit it to the registrar’s office of each institution attended 和 instruct them to enclose the form with your official sealed transcript. Transcripts must be sent to the 《十大菠菜靠谱老平台》 Transcript Department. 《十大菠菜靠谱老平台》 accepts official transcripts sent directly from the registrar’s office only. - On the 《十大菠菜靠谱老平台》 application, enter all coursework listed on your transcripts, not just the prerequisites. If you do not wish to enter each course, you have the option to pay the fee for Professional Transcript Entry Service (PTE) offered by 《十大菠菜靠谱老平台》.
- Pay the initial application fee when submitting the 《十大菠菜靠谱老平台》 application.
- 在《十大菠菜靠谱老平台》, identify the three (3) contacts from whom you plan to request letters of recommendation/evaluation. At least one (1) of these contacts must be a Certified Athletic Trainer (ATC). These evaluations must be submitted through 《十大菠菜靠谱老平台》, 和 should attest to your academic ability, 职业道德, 和 potential to be successful in an athletic training program.

- Submit documentation of at least 50 hours of volunteer service, 阴影, or interning with a Certified Athletic Trainer (ATC). 其他需求
- Three (3) evaluations/letters of recommendation (see 《十大菠菜靠谱老平台》 guidelines for submission). At least one evaluation must come from a Certified Athletic Trainer (ATC).
- Fifty (50) hours of volunteer/shadow/intern experience with a Certified Athletic Trainer (ATC). Hours must be self-reported in 《十大菠菜靠谱老平台》 和 documentation of hours with ATC signature must be uploaded as supporting documents.
- GRE scores self-reported in 《十大菠菜靠谱老平台》 (GRE taken within past 5 years). *Any applicant who has difficulty obtaining a GRE testing date or meeting the required 50 hours of observation due to COVID-19 should contact Dr. 巴拉德(jballard@hawks.十大菠菜靠谱平台.edu).
- Qualified applicants will be invited for on-campus interviews once applications are verified in 《十大菠菜靠谱老平台》.
- Currently, the MAT program is unable to accept international students.
Provisional/Conditional 入学
An applicant whose undergraduate 和/or prerequisite coursework is in progress at the time of application 和 will be completed before beginning the Huntingdon MAT program may be offered provisional/conditional admission to the program. Any offer of admission is contingent upon successful completion of an undergraduate degree (minimum 3.0 GPA) 和 prerequisite coursework (grade of C or better in each prerequisite course).

Enrollment Confirmation
Students who are accepted into the program will be required to pay a non-refundable deposit of $300.00 confirming their intention to enroll in the program. The fee will be due within 14 days of receiving a letter of acceptance into the program. The deposit will be applied toward the first year MAT program fee.

Upon admittance into the program, students will be required to complete the following:
- Students will be required to attend orientation the week before classes begin.
- During the orientation program, students must show proof of immunizations 和/or waivers of immunizations.
- During the orientation program, students will undergo fingerprinting, 背景的测试, 药物测试. Failure to complete these activities 和/or inability to successfully pass these screening procedures will result in dismissal from the program.
- Students will be required to become certified in CPR/AED for the Professional Rescuer as a component of the program.
- HIPAA certification, OSHA Training, 和 bloodborne pathogen training will be conducted onsite. The student must complete training 和 pass all necessary assessments in order to meet this requirement.
- Students must show proof of health insurance.

- Students must sign a confidentiality 状态ment for athletic training students.
- Contingent upon institutional policies, students may be required to provide proof of a negative COVID-19 test prior to arriving on campus. Students will abide by all federal, 状态, 和 local ordinances as well as institutional policies regarding use of masks, 社会距离, or other infection control measures.
